Why Can't I Get Motivated

I want to go kayaking; slip through the ocean quietly and peacefully. I want to go camping. Pitch a tent, build a big fire, stare at the stars and listen to nature's peacefulness. I want to go for a bike ride. I want to go for a hike. I want to paint three paintings. I see them in my mind. I need to get them out of me.

I sit and stare at the wall. I want o change. Really I do. I want to be different. I want to DO things. I want to be active. I want to be busy.

The dishes pile up, and up and up. The laundry pile gets bigger and bigger and bigger. I feel completely overwhelmed by just the ordinary responsibilities of life. I feel overwhelmed even though I barely do anything.

I get up out of bed. I walk the dog. These are the two consistent things I manage to do.

Mostly, I feel suffocated by my existence. I feel ashamed for how little effort I put into helping myself get better. I feel like I waste Dr. X's time, because I know what I need to do. I need to DO. I need to get active. I need to participate in life. I need to work, to contribute.

Why then, do I just sit there staring at the wall for hours on end wishing my life would change?

Do You Ever Feel...???

(Please note: I the following post I use the words, "crazy, insane, mad etc. loosely in describing only myself. I in no way intend, or even ever feel, that others having similar symptoms are anything but beautiful, though at times tragic), examples of the breadth of human experience). I'm just exhausted and weirded out and scared, and probably bitter and angry, about my own experience)

Do you ever feel like it doesn't really matter how you feel, because you are fucking insane, and everyone thinks you are crazy, so no one "really" listens anyways? And even if they did, the way you are would make absolutely no more sense to them than it does to you?

I feel like I am losing touch with reality. Yet, how can I be losing touch with reality if I know I am losing touch with reality? I am seeing more and more things around me, and inside me that are passing me some kind of meaning filled message/s...(though can't quite figure out the message/s...), that are making me feel like I am being sent messages, or things are showing themselves to me in order that I find some deep meaning in their presence.

(Eg., mood switching up after going to the front of the church and praying every night, mystical animals (crows) appearing when I need them to appear, pictures of wolves falling off my bookshelves, another wolf appearing on a torn out page of a children's book, right in front of me on the sidewalk, and then the terrifying wolves showing up in my nightmare, then the number 3: three pictures, the 3 nightmares, each nightmare made up of 3 parts)

Thing is, although my mind really does believe this "messages are being passed to me" idea pretty deeply...I can also see that maybe I'm crazier than I thought or something is happening whereby things just "happen", or "show up" at truly random times and places and for some reason my brain is seeing all these things as signs or patterns of meaning that I am MEANT to see??? God help me...

Since I got out of the hospital I have been severely depressed, almost more severely than when I went into the hospital...except last Wed evening my mood started to rise and I had a really great week. All week the rapid onslaught of negative and suicidal thoughts completely stopped, and I felt hope again. I felt like I was cared for, had tons more energy, felt talkative and social. I pretty much felt like I was on the mend. In fact, I felt sure I was becoming well.

...Until Sunday at 5pm.

You read that correctly.

Somewhere around 5pm on Sunday the voice inside my head returned with no warning and no reason behind it returning. {note: Although for the two nights prior, I had really two really bad nightmares, the likes of which I haven't had for a long time (and last night my wolf returned to me in another nightmare}

Just as I was entering the 5 rd bridge the voice/thought, (It has no sound...so it is not really a "voice", but it seems external), said to me:

"You have failed everyone. You have disappointed your dad so much, what a waste of life you are". And I started to cry.

Ever since then the voice has been going on and on about every bad thing I do, or mistake I make, how everyone really is sick of me, how I am not really loved by my boyfriend, how he is pulling away from me, how I have disappointed him and destroyed our relationship, etc.,

Then the voice switches to telling me how fucked up and wrong my thinking is, and how I am just imagining all this stuff and if I would just stop thinking it would all go away...but either way the voice goes on and on and on...maybe it is two voices?? I don't know anymore what is real, and what is my brain having crazy talk.

These thoughts/voices intensify and speed up or slow down. I believe they are magnified when my anxiety gets worse...as I have been having really bad anxiety/panic attacks. It may be though that the voices/thoughts are causing the anxiety as they begin to race faster and faster, and seem to be pushing me to change or die...whichever.

Either way there is an OCD like quality to them...including having a song I have recently listened to play relentlessly over and over and over in my head, as a torturous accompaniment to the thoughts that continue, and often race, faster and faster, harder and harder, over, and over and over, and over. Unstoppable. Unrelenting.

Sometimes I get so much "bad energy" inside me that the voice starts to speed up and it talks at me, over and over and over, at mach speed; incessant, punishing, shame inducing, and guilt producing. Often the voice has suicidal plots and plans, and reasons to die . Lately this is much much worse than any time I can remember before. It is like the thoughts are trying to push me.

If I wasn't crazy before...something inside me is aware I am crazier now. This is not right. I can see I am having strange thoughts...but my brain cannot seem to stop thinking (and while I am having them), believing these thoughts. Please, help me!!! Am I completely losing my mind?

Catch Up

I have been in the hospital so haven't been able to keep up with my postings or responses. I apologize. I am still "technically" in the hospital, but am out on a pass right now. It feels so good to be at home!!!

This post is going to be a smattering of things I have been doing, information about treatments for depression I have been receiving and results I have experiencing from those treatments. Here goes...

1) ECT (electroconvulsive therapy)...I remember, years ago when I was in university, the student newspaper published an whole issue that was radically anti-electroconvulsive therapy (ECT). Am I ever glad I never listened to all the negative hype and (with trusted support and help) decided for myself whether I would try ECT as a treatment.

I have been receiving unilateral ECT treatments approximately 3X/week for 3 weeks. The X-mas holidays made it a bit mixed up. For the past couple weeks I have had ECT treatments once a week. I said in the previous post my increase in mood has been nothing short of miraculous. I was so severely depressed a couple weeks ago, and throughout most of the past 17 plus years( ...actually I had MDE's even before that, but the worst ones and this past 10 year plus treatment resistant MDE, have occurred in the past two decades).

Yesterday and the day before my mood was so intensely high I felt like I was in heaven. My self had come back!!! I was bubbly, effusive, effervescent, talkative, extremely outgoing, energetic, so happy I cannot even describe it. I felt like I was glowing, beautiful, and I felt extremely sexy and sexual. This is how I feel when I feel well. I think the intensity feels so strong because feeling good for me has been so hard to achieve, and suddenly all my bad feelings and thoughts were gone.

Unfortunately, my mood began to crash last night, but nowhere near as low/bad as it was before. It definitely is low though...which is upsetting, as I have seen this kind of cycling mood before, where I become my high self for a few days, only to crash and burn a few days later.

I am hoping this crash is temporary, and caused by my not sleeping the other night. I slept last night, and hopefully will sleep again tonight, and maybe that will help lift my mood again.

2) I don't even want to write this, but I will...I wonder if I have been hypomanic the past couple days. I really did not think so, until my boyfriend and I started discussing my behaviour and he told me I was being really pushy about sex. Until he said that I just felt like me and my "true" high sex drive had returned.

I know I have a much higher libido than many women...even when severely depressed, if sex is initiated by someone else I almost always am happy to participate...though I have difficulty initiating when depressed). I was feeling extremely sexual last night, and I can see how my boyfriend might have felt a bit overwhelmed by my advances, as he was not feeling very well.

He told me that sometimes when I am "high" I get really demanding and pushy. I have received this feedback from other people before, that sometimes I am so "ramped up" and "on", and when I am I sometimes demand the same level of energy and competence from others. I have seen this in myself too. I can tone it down, but unless I constantly pay attention to my behaviour I tend to slip right back into high energy mode.

Problem is...I think this behaviour is my personality...not my mood. Many members in my family have this same personality...super on all the time, constantly busy and always doing many things at the same time. My Dad is like this, as are both my sisters. I am a bit more outgoing, talkative, and effusive than them, but my paternal grandmother was EXACTLY like me, right down to being easily annoyed by anyone who couldn't keep up with her.

I am not saying that I am proud of my negative behaviour, but unfortunately I think my pushiness is tied to my "high" personality. In my defense...unlike my Grandma...who could really hurt others feelings with her insensitivity, I am really sensitive myself and knowing how easy it is to feel hurt by someone else, I try really, really hard to not hurt others, or effect them in a negative way. While sometimes I do make mistakes, or forget to hold back; I work hard to be as thoughtful, and aware of how by behaviour affects others as I possibly can.

There is more to write, but I need a break...I will continue to write again soon. Take care everyone.

P.S. Thank you so much to all the friends, readers, and health care professionals who have helped me throughout my depression. Of course, an especially big thank you goes out to Dr. K. (my psychiatrist). I would not be here without him. He is one of the most caring, knowledgeable and consistently supportive doctors and people I have ever had the pleasure to work with. Thank you so much for never giving up on me.

This needs to End...NOW.

I am scared of what is happening to me. My mood is cycling all over the place and the switches are shifting suddenly into either my behaving in weird ways, or I suddenly become so intensely and severely depressed I fear I might hurt myself.

I woke at 7am this morning. The second I woke I felt that black pit of hell in my stomach. I was so depressed I felt physically ill...and it got worse. At 7:30 I was trying to make a coffee, and I picked up a knife (the only thing close) to stir the water and coffee in my french press, and suddenly I had an intense urge to end it all; to stab myself over and over 'til it was all over and finally I was at peace.

It felt so urgent; as though stabbing myself was my destiny. I stood there frozen, helpless to stop myself, and the urge kept getting stronger and stronger. I felt compelled to do it.

I don't know what happened because one minute I was feeling compelled, and the next I was crying. I sat down and stared at the wall, unable to manage even a thought. I sat there so tired/fatigued I couldn't move. I sat there dazed and empty, in some kind of void...maybe in a dissociative state??

Even at 8:00 am it was so hot and humid. I wasn't feeling safe at home, but I had no one I could call. It was too early in the morning. I pulled myself out of the chair about a half hour/hour later, dragged myself and the dog out the door, and went to the ocean for both of us to cool off. It took every single ounce of my energy to get up and leave the house.

I sat on a log and cried. I felt, and feel, so lost; so completely and utterly confused, hopeless and unsure about what is happening to me.

Just the day before I was feeling pretty okay. Maybe a bit better than okay. All day the day prior, I was extremely sexually charged. To the point where I just could not get enough. When my boyfriend left for home I started looking at porn on the internet...and did so for hours, until 1:00 pm.

I know I have expressed my having watched porn before, but, but then, as was the case yesterday, this both is not "typical", or "normal" behaviour for me. Also, I have never watched 7-8 hours of porn before. It's just lately I have this insatiable sex drive and I can barely manage it. There is more to this behaviour, but I am too embarrassed to write about it. Surprisingly I do have a modicum of modesty left in me.

So last night I am okay, and this a.m. I sank so low I didn't think I could make. This has been the type of cycling that is happening, and I can't handle it anymore. I don't know what to do, because at this point, 8 years into this episode, it really does look like here lies my destiny...to be completely out of control of my mood for the rest of my life. I can't handle that.

Random Shapeshifting

I have been trying hard to figure out what happened to me on Thursday (and the few days prior). Thursday though seemed to be the day I was pretty sure I couldn't go on. Why then, did I wake at 3am on Friday in a completely different mood state?

Throughout the day on Thursday I made myself crawl into bed both to protect myself and to relieve the intense fatigue and anxiety (is it possible to feel both???) I was feeling. By the evening I was spiraling into such brutal suicidal thoughts that I didn't think I was going to manage to survive them.

Nothing happened between 11pm Thursday and 3am Friday. I had not read my blog and the supportive comments on there. I simply had switched into an okay mood; not high, but okay.

I do not get it. How am I supposed to work on changing myself to help my mood, when it really appears "I" have no say in the mood I have any given day? If I have no control over what affects my mood, why even bother with therapy, or trying to change, or doing any of the things to help myself change?

For me, mood changes appear to be random. Is that strange? Do other's moods switch in a similar manner? Am I the only one who has this experience? It is disconcerting to have such little control over one's self.

Cycling Downward

Sometimes life seems like a something I am forced to endure. I am, once again cycling rapidly down into hell despite all the help, medications, support and treatment I receive.

There have been so many times when I have believed euthanasia would be the kindest treatment. In the paper yesterday there was an oped piece about how people who believed in euthanasia were stupid. Her argument was based on someone saying an 18 year old depressed person should have the right to die. The writer wrote the piece as though it were impossible to be rational and hold this position.

I kept thinking how clearly I could see where this might be the kindest treatment for some 18 year old people, along withsome 20, 30, 40, 50 year oldpeople and older people as well. It doesn't come down to age for me, but for suffering.

I can understand how an 18 year old might have gone through so many treatments, attempts to help them, and nothing working. I kept thinking the woman writing the piece has no clue how painful depression can be or how it can destroy your soul and all your hope and desire to live..

Even when I feel okay there is always an underlying desire to die. Certainly, even when I become high I would understand if the other side of me ended it all. Life is just too hard to manage sometimes.

When I began psychiatric treatment, and when I was in therapy all the times before that, I expected to get" "better". My definition of better requires I become well like I am when I feel "right and good" AND that I stay that way forever.

It used to happen. I used to become better for really long periods of time, sometimes a couple years at a time. That created hope in me. It made me think that it was possible to live a normal life, to want to keep trying.

I think the reality is now that I need to accept I will cycle forever; accept my mood will go up, but crashing eventually is a certainty. So why bother trying? why stay in treatment? Why take medications when you aren't any more protected from your depression than without them. I am really wondering if it is worth trying anymore.

Be Embarrassed and Do It Anyways

Today, I woke up depressed, flat, apathetic, a feeling of dissociation throughout my mind...detached, disconnected. Alone.

I met with my friend "Z" and went for a late breakfast and then she took me grocery shopping... a task that I dread and fear when I feel like this.

As we sat having breakfast I felt so cared for and understood by her. I was going on about some "bad girl" stuff and she told me I was like a train...impossible to stop. She meant it as a compliment...like, even when depressed, I have a powerful joie de vivre and head straight into everything I do at full blast straight ahead.

I really like that imagery of a train plowing where it is destined to go, or wherever it desires to go. It made me feel like my old wild self. It made me feel valued for who I am. It also made me recognize I am strong and powerful and there is no stopping me once I get going. Getting going is the hard part.

I have had an awful cold/sore throat for a few weeks now. I cancelled my singing class last week because I didn't think I could sing. When I walked into Dr. X's office and told him I cancelled he suggested I reconsider. He did so for a couple of reasons. He said I could still learn things other than the singing aspect of singing...things like timing or style. He also he wanted me to challenge my fears about making mistakes.

I was afraid to go to my singing lesson because I had completely blown a solo in choir the night before. I was so embarrassed...mortified might be a better word, and I felt sick about my mistakes. He told me that feeling the embarrassment and doing it anyways was a great form of behavioural therapy. This was a great opportunity.

This morning when I woke up depressed and still sick I kept trying to tell myself I shouldn't go to choir practice tonight. I tried to nap and couldn't and as I realized why I couldn't. I clued into the fact that it was not because I felt sick, rather I was ruminating about the mistakes I had made last week. I was AFRAID to go.

I realized this is exactly when Dr. X. explained to me I need to go (which I did). Go, and even make the same mistakes (which I did) and survive the embarrassment (which I also did...though I feel sick about it again)...at least I went and tried.

On my way home, despite my embarrassment and some difficulties feeling like I was "outside the circle" or detached from the group, I felt really good about all I did and accomplished today. I felt good that I recognized a pattern and challenged it head-on. I didn't back out of or cancel because I was depressed. Because I was powerful and challenged myself my mood lifted and I sang to my favourite songs all the way home.

Gone

The Lady of Shallot, John Williams Waterhouse, 1888

It is gone. My good mood has disappeared and suddenly been replaced with exhaustion, fatigue, a desire and need to sleep all the time, depression and all its accouterments: fear, anxiety, dissociation/depersonalization, sadness, crying, an intense sense of loss, fleeting thoughts of my suicide and a fixation on other's successful suicides.

Just yesterday I wrote of creating a "benevolent structure..., especially when depressed". I could feel my mood slipping last week. I woke one morning and it was gone. That excited, life seeking, experience focused me was suddenly replaced by my old self.

I thought I had healed. I thought I was becoming well. I thought my depression had finally ended. I thought a new life had opened up for me. No such luck. How can I be so stupid, so mistaken, so wrong?

Last week I asked Dr. X if I would continue cycling my whole life. Usually he says we will find something to help me...and I always think he is lying to protect me. This time he said, "Maybe". I have long thought this to be the truth.

I am trying to live despite this being the truth. In fact, I have thought that to be the truth and to try to help myself I have fairly recently tried to really, really accept that idea: the idea that I must learn to LIVE with depression and swift mood changes. In keeping with that idea I am really trying to create a benevolent structure that works for me even when I crash, even when all I want to do is sleep.

It is really really hard though, to keep going when I feel this depressed. Today I have volunteered to teach a class. All night I dreamt of how I was going to mess it up, forget my material, be late for the class, be confused and completely disorganized while I am teaching the class. feel sick about having to teach. However, because I have made a commitment to teach I will go fear and anxiety or not.

I am not so sure that is a good thing right now. I have made so many commitments over the next few weeks that I am exhausted and anxious (go figure...how can I be both?) just thinking about all the things I need to do. Unfortunately, or fortunately, almost everything I committed to involves other people who are relying on me to show up.

Ironically, my benevolent structure is working as it should be, but I am not sure I want to do these things anymore. In my recent posts I have tried and tried to convince myself that doing things is the right thing for me to do. I'm not so sure that keeping up is possible now. How do I keep up with my schedule when all I want to do is sleep? When I am afraid to do the things I said I'd do? When all my will and motivation to do these things has disappeared into thin air?

What do I do now that I have become so depressed again?

Who AM I?

Today has been a strange day. I have felt my mood going all over the map, from feeling joyously happy, to grieving the loss of, and missing, my Mom (I felt her near me, and had reminders announcing themselves to me, all day today...it began to feel eerie and frightening...like I was having a premonition).

I was very anxious most of the day; so anxious I could not breathe properly most of the time. Even when I felt joyous, I was also feeling really, really agitated, or intensely filled with energy.

I felt paranoid I am being spied on numerous times today. I feel lonely; alone, uncared for. Some moments during the day I felt relief I have been feeling better. Some moments I felt the fear that tells me I am slipping into my old ways.

When I say I feel agitated/energetic it is hard to tell which it is. I feel compelled to move, to tap my fingers, to tap my fit, to move my toes, to squirm and fidget. There is some force inside me pushing me to constantly have some part of my body tapping out rhythms at a quick pace. Drumming, tapping, tapping, drumming, squirming, swaying, knees moving up and down...annoying, but I can't seem to stop. I feel wired in the mornings and early afternoon and tired in the late afternoon.

All day I was "obsessively" singing. I say obsessively because I couldn't stop. I kept having a song go through my head, and felt compelled to sing it, over and over and over and over and over...ad infinitum. "Willow weep for me, willow weep for me, bend your branches green into the stream and cover me, listen to my plea, listen O willow and weep for me." I kept singing those lines over and over all day. No matter that I wanted to stop, it was like I was stuck on repeat. This seems similar to when music was stuck in my head before when I first began Prozac.

Then I have a strange noises in my ear/s. It's hard to describe. sort of a crinkly, tinny, sharp sound that happens beats or clicks loudly for a few seconds then disappears. It is happening in my left ear, but I think I heard a similar sound in my other ear yesterday...but maybe I am mistaken and it was the same ear. Its really high pitched. Almost like a crickety sound. It scares me when it happens, because it seems like it might stay, or not go away.

Speaking of going away...I feel lonely, not because I am literally alone as my husband is away for the week. I feel lonely and alone because he's been gone since Saturday and he hasn't called me. I am not really one to talk too much on the phone, or even to expect my husband to call me, but for some reason his not calling makes me feel unimportant. When I go away for a week I usually call for a few minutes every few days, just to tell him I love him and am thinking of him. I think there is something about not having that kindness returned that is urksome. Is it too controlling of me to want a call from him when it's only a week?

Sad thing is I am actually glad he is away. The break is good for me. So not sure why I care if he calls. It is amazing how self sufficient I am when I'm by myself. Without him telling me I'm a loser all the time, without him pointing out all the mistakes I make and all the things I don't do well, without him trying to control my every move consciously, or unconsciously, without him trying to force me to do the things he wants done, I begin to feel I actually CAN do things, and AM good at things.

So...happy to sad, to grieving, to anxious and so agitated I can't stop moving, to angry and frustrated and lonely...to okay again. My mood is as labile and unpredictable as the weather in the city where I live. Who am I? What do I feel?

Soaring

I am really "over the top". I can see I'm going to fast, but I cannot slow down. It is strange, but in many ways I feel like myself again. I am ebullient, chatty, charismatic, and "on". I have tons of energy and feel really good. However, I can see I might be a little annoying too.

I am catching myself talking to loud, too fast, over speaking people, talkling on and on to people I don't even know (people on the ferry, at the store, etc.) ,interrupting, and too enthusiastic (yes, that is possible). I can see I need to back off a bit, but I try and then I do it again. I both hate and love this side of me...the bursting with stories is good, until I find myself dominating the conversation. My energetic self is good, until I find that energy turning into anxiety, irritability, or worse yet, agitation. My effervescent self is good until I overwhelm people with my thoughts and ideas. My energy to do things is good, until I plan a trillion things and manage to do none.

All in all, while I wish I were a bit calmer and quieter, but I certainly would rather be this way than my depressed self. Right now I want to LIVE. It has been a long time since I have felt up for more than a few days here and there.

I slept last night...I think because I took 1000mgs Tegretol and 100 mgs Trazadone, but now...at 12:14am I am wide awake and can't get to sleep. I have to get up early tomorrow too. Yikes. Here's hoping my energy will subside ASAP (for the night) and will return with the sunshine. (Well the fog actually...it is thick and solid right now...scary to drive in)

I Am So Tired of Trying to Get Better.

(All photos by Aqua, November, 2008) These first two are the strangest mushrooms I have ever seen. At first glance I thought it was dog poop plopped on top of a stick, until I looked at it from ground level at the side (don't ask WHY I would do that if I thought it was poo!!!). When I looked at it from "mushroom level" it looked like something out of "Lord of the Rings"

I wanted to go into my appointment today and focus on all the things I DID do this week. I wanted to try to shift my appointments away from my morose and markedly anxious and depressed mood. Last week Dr. X. said we do not have to talk about my symptoms, or my depression in our sessions. We could have appointments where we just discuss other things and have a conversation about whatever I want.

At first that seemed weird. Why would I see a psychiatrist and not talk about my psychiatric problems. As the week wore on I began to think maybe that would be a good idea. Maybe if I focus on the things I enjoy, rather than the thing I least enjoy I would eventually forget I'm depressed (hey....I seem to forget everything else!).

I tried really hard this week to find pleasure in simple things. The most enjoyable moments were spent with my puppy, in the forest taking pictures of mushrooms.

Mushrooms? You may be REALLY worried for my mental health now,.but I cannot get over the diversity of these little things. I also cannot believe I never noticed all this variety of fungi sitting right in front of me my whole life. It is strange how when you start to look for something it is as though a magician has placed it everywhere in front of you .

Perhaps I should begin looking for, and recording, enjoyable moments. Maybe they would become as prolific and diverse as all the mushrooms I have been finding. Maybe eventually the damp, dark forest that is my mind would eventually create joy as abundantly and easily as the real forest breeds mushrooms.
Maybe each enjoyable moment would create a path in my brain, and finally all those paths would become happiness, or at the very least, the absence of depression.

Probably not. More and more and more and more and more etc. I sense this depression is never going away. I sense my Mom was right: I need to accept my depression and move on WITH it. Maybe some of us just never get better. Maybe some of us are destined to be depressed with only fleeting moments of feeling okay, or experiencing joy, insight, or happiness. Can I accept that as a life worth living?

Some may say I should feel blessed that at least I have moments of levity. Some people do not even get those moments.

I don't feel that way. Those ethereal moments almost feed my depression. I say this because the joy makes the sadness even more evident and profound. Every time the joy, the laughter, or the connection I feel with someone, or something, ends I feel even more depressed. Those short experiences of a lift in mood seem to be there only to show me what I am missing. While photographing these mushrooms I felt a sense that they had more of a meaningful and purposeful existence than me.

I am sorry that some people have no moments of joy, but I am losing patience with having no mood stability. I want the old me back, the working me, the proud me, the ideas me, the fun loving me, the free me, the gregarious and mischevious and loveable me. That said, I am pretty certain the old me is gone and this is the new me. If that is the case, I really would like would like this life to end ASAP. I'm not cut out for this anymore.

I Fear I Will Be Scrutinized

It is difficult to recreate the intricacies of a therapeutic moment, or moments when I come home, and sit down to write a post. Often, even within the therapeutic moment, it is difficult to describe, explain and sometimes, even bring up, what is going on inside my head. That is how I am feeling right now.

Dr. X. is so patient and understanding when I am trying to get things out. We have a long therapeutic history together, and on a huge level I trust him more than anyone on earth. Intellectually I know he is trustworthy. When it comes to an emotional level though, I have a difficult time trusting that anyone is who they appear to be.

His behaviour has always been consistently kind and compassionate. He has always appeared to care for me and not judge me in any way. In fact, I cannot think of a single time when, (factually vs. my paranoia) he was anything but supportive. I do not know why I cannot always believe he will not reject me, or does not secretly dislike me, or does not privately disrespect me, or think I'm a complete waste of space.

Most often I feel like my even thinking he would be like the person I worry he might be is disrespectful towards him on my part. He has never given me any indication he thinks anything but good thoughts about me. It is me who has difficulty believing; especially when I am really depressed.

I am using "belief "in the philosophical sense, where it is an unquestionable belief that leads to knowledge. An example might be believing the world to be round, allows you to take that next step and try to sail to the other side, whereupon a successful sailing, where you did not fall off the edge of the world leads to clear knowledge that, at the very least the world is not flat, and your belief it is round is closer to becoming knowledge.

This might be part of the problem. A patient can believe anything they want about their psychiatrist, but there is never any opportunity to have those beliefs turned into knowledge. You can never "know" your pdoc, you can only believe what you experience. To feel safe I need my beliefs backed up by knowledge.

The therapeutic alliance is such that the patient knows very little about the psychiatrist. In many ways this is a beautiful thing. For me it means Dr. X. can be anyone I need him to be at any given time. On another level it can be a frightening thing, especially if paranoia, or fear of rejection, or an intense need to be cared for, or valued, or not rejected are symptoms that flair up and become intense when I am depressed. Unfortunately, when I am really depressed, it makes sense that Dr. X. becomes the subject of my fears.

Today we reviewed my schedule. I did not do very well. In fact the things I really feel NEED changing did not get done at all. This led to the following paraphrased and (very) abridged recap of our discussion:

Me: I hardly did anything this week.
Dr X: You made up the schedule and came prepared with another schedule for this week
Me: Well, I haven't even filled the new schedule in yet and I didn't dust, or drop off the laundry, or cook dinners...yadda, yadda, yadda...I am so disgusting. If you saw how messy and dirty my house was you would be disgusted.
Dr. X. I challenge you to take pictures of your messy house and bring them to show me in our next session.
Me: (inside feeling absolutely mortified by the thought of doing that.) There is absolutely no way I could do that.
Dr. X: Why do you say that?
Me: (inside thinking a hugely complex set of ideas...I'm disgusting, it would destroy any semblance of "normality" I have tried to maintain with Dr. X, I would completely destroy any image of "togetherness", or likability I have left with him, etc.) I am afraid you would not like me, or would disrespect me.
Dr. X: Maybe this is an important behavioural exercise. You keep thinking I will not like you for your messy home. Showing me your home and seeing your worst fears be mistaken would be a good exercise.

This is when I began to fear my whole good relationship with Dr. X. was just imagined. I knew nothing about the man except what I see in our sessions. I imagined him having a beautiful and immaculate home. I imagined how disgusting my home would appear to him. I believed if I showed him my home he would be disgusted with me. I believed that I had worked so hard to be "likeable" and pictures of my house would destroy all I had worked so hard for.

It became obvious to me that showing Dr X. my house would lead to him disrespecting me, seeing how truly lazy and unorganized I really was, and that inevitably it would lead to his rejection of me as a person he valued.

I understand that taking the pictures and "joyfully" sharing my messy house with Dr X. could be good behavioural therapy, but what if it backfires and he really is disgusted in me?

Please note: I understand this trust issue is my problem, and that Dr. X. has never indicated in any way that what I fear would take place, but the fear is so powerful I feel sickened that I even tell him this stuff.

I just saw(seconds ago) this very fitting video...

Hope Does Not Stick Like Glue

Maybe you noticed my mood fluctuates often, maybe you didn't. When I am in a mood state it feels like it will go on forever. It never does. I have glimpses and a teeny bit of insight that maybe I will crash when high, or feeling well. When low I really have a hard time seeing it will ever change.

This morning I replied to "S" that there is hope. A few ays ago I wrote about my new found discovery that hope exists. For some reason this afternoon that all seems to have dissipated.

I feel exhausted. I have a class to teach I am dreading going to. I have slept for the last two hours. Hoping the day would disappear.

Yesterday my Uncle (on my Dad's side) brought the last of my Grandma and Grandpa's friends still alive out to see me. They were my grandparents closest friends and I have fond memories of them. We had a very short, but good visit. It was good to see them, and nice of my Uncle to bring them to my city to see me.

However, as he left he rejected a hug I tried to give him, while U was saying thank you,to him. To top it off his last words to me were, "Don't work too hard".

He knows I am not working. In fact two weeks ago I sort of "came out" to my aunt and uncle about having panic and anxiety problems...though I never mentioned that was why I didn't work. The day before yesterday I told my cousin I had a mental illness and explained a bit more to her.

We had planned for my Cousins and Aunt and Uncle and my G'mas friends to get together, but only my uncle and G'ma's friends showed up. I think it is because they see me as a loser now. I believe my Uncle's comments were meant to hurt me. He is an intelligent man . There is no way he could not have understood that what he was saying was sarcastic, and that I would feel hurt by that.

So right now I feel like a loser for not working. I feel like he saw me as completely able to work; like he was testing my comments that I have a difficult time in big social gatherings, because I declined to go to my cousin's wedding due to my social anxiety.

Yesterday we were at the casino, but it isn't social gatherings where I don't have to interact with a large number of people I fear. It is gatherings where I hardly know anyone and am expected to be social. I'm usually fine with a small group, or somewhere with lots of people where there is no expectation I interact with anyone.

Anyways, maybe there is hope, but it waxes and wanes with both my mood state and my resiliency around rejection and authority figures. I am crying right now. I really hate this illness. I do not want to go to my class. I feal really stressed out about it. So "S" if my note this morning sounded too optimistic...right now I'm taking it back.

Hell Disguised as a Blessing

There is something to the Bible's old testament story of the beguiling snake coaxing Eve to eat the from the fruit of the tree of knowledge in spite of her love for her freedom in the garden of Eden.

I feel like Eve after she has eaten the apple whenever I receive a letter or inquiry from my disability insurance company; Naked, overexposed and afraid I will be cut off from receiving support while I try to get better.

My mood has dropped immensely over the past two weeks. I cried on my way to my appointment today. I cried much of my appointment today, and most of the way home.

To top off a bad morning, when I got home, I found a letter in my mailbox from my insurance company wanting to know EXACTLY how I was, WHAT I had been doing and for how long, what I as able to do, who I see, if I am volunteering or working, at what and for how long, what I participate in, and sign the note at the bottom saying we can have access to all your doctors notes, thank you very much (my paraphrasing of the received letter).

This letter put the fear of God into me (only God is spelled "losing money to support myself before I am well enough to do so on my own").

How do I fill this form in? I am so conflicted because my mood is so unpredictable. If you had asked me 3 or 4 weeks ago I would have been positive I was getting better, last week not so sure and this week, back to square one.

I volunteer 5-6 hours a week (2.5-3) I teach beginner's drawing, the other I show up on a specified day and help people in the painting studio if they need help (often I just show up and do my own art and no one needs my helps, but I am there if they need me.

So I struggle. I think to myself, "well, if I can teach an art class, why can't I go back and teach a banking class? what's the difference? And I am really afraid that is how the insurance company thinks?

The problem is at the Art Clubhouse I put my best face forward (just like I did at work) and I am professional and outgoing, and reliable...no matter how I feel inside, because I feel that way is my responsibility. At work I never let my illness impact my work until the end when I was so sick I couldn't do anything but stare at my computer screen. Even then, because I was so reliable for so long, my boss never even noticed me crashing and burning.

What the Occupational Therapists and staff at the Art Clubhouse don't see is that spend all morning trying to get to the studio by 1:00pm and when I leave and get home at 4pm I am so exhausted I sleep until 6 and sometimes 7 or 7:30pm. I do this everyday, even if I've done nothing all day. I feel so exhausted I feel sick.

I cannot manage to clean my house. I have not been cooking anything other than pre-made dinners, or smoothies, or eating cereal except maybe once a week or even less, when I somehow find the energy to make dinner. Even when my mood was really good I could not get these things done.

I am worried that the insurance people will translate my being able to volunteer 5-6 hrs a week, and take a guitar class, and paint sometimes as a sign that I am well enough to return to work. I know I am not. I know I can only do those things at the studio because of the type of supportive and safe setting it is. I know I can only manage that volunteering because I know that when I get sick (which I always do) I have the option of backing off on any commitments.

Anyways, I feel sick about having to fill out this form. I don't even know where to begin when some days I soar, and others I want to stay in bed all day. This past two weeks I am nosediving into the latter scenario.

I wish I had not accepted the fruit from the poisonous tree. It ties me down, frightens me, limits my ability to try new things and makes me feel like I am doing something wrong by volunteering, makes me feel guilty for accepting money while I am not working. I am constantly worried I will lose my insurance before I become well enough to work again and take care of myself.

I wish I had been able to quit my job when I became ill. I wish I had not needed to rely on a bureaucratic organization, one that scares the crap out of me, to survive.

Two Wolves

Last Sunday I went with two friends to a local Community Centre to participate in something called "The Remarkable Women's Poster Project". We each brought a photo of someone we considered to be a remarkable woman and made a poster of that woman. These poster's will be on display at the Community Centre to celebrate National Woman's Month in October. I did one of my Mom.

If you can believe it my best friend did one of me. It was so beautiful. With my permission she "outed" me as a person with a mental illness. Her caption below my photo read: "My Name. Still living. Yeah!" I felt honoured that she chose me as a remarkable woman. Thanks "H".

At the Community Centre there was a poster pinned to the wall with a fable written on it. I do not really subscribe to the pejorative nature of the descriptions of evil in the poster. I am not even certain I believe in "good" or "evil".

I also recognize that Major Depression is the instigator of many of the "evil" symptoms below, but I also think that despite being severely depressed I can push a tiny bit to rid myself of ego and it's soul impairing accoutrements.

Given my propensity to dream about wolves and to cross paths with coyotes the poster captured my eye. This is what it said:

"Two Wolves:

One evening an old Cherokee told his grandson about a battle that goes on inside people. He said, "My son, the battle is between two 'wolves' inside us all.

One is Evil.
It is anger, envy, jealousy, arrogance, self-pity, guilt, resentment, inferiority, lies, false pride, superiority and ego.

The other is Good.
It is joy, peace, love, hope, serenity, kindness, benevolence, empathy, generosity, truth, compassion and faith.

The grandson thought about it for a minute and then asked his grandfather,"Which wolf wins?"

The old Cherokee replied, "The one you feed". "

I am so afraid my improved mood and my new found capacity to participate in life will be taken from me; that my mood will disappear into anguish, like it always does. I am trying so hard to feed the "good" wolf, but am terrified that the "evil" wolf gently slips back into my mind unnoticed. I am trying hard to remain vigilant and push forward while I feel well. The thought of returning to the place from whence I came terrifies me.

I SHOULD Have Stayed in Bed...

I had my pdoc appt this morning. I should have stayed in bed. I sat there wondering if I should continue therapy, continue medications, continue any of this. I sat there feeling overexposed and under protected, listened to, but unable to say what I feel...partially because I feel nothing and partially because I feel like I'm just biding time until I die.

I see no real change in my perceptions taking place. A friend told me about an exercise she was doing, whereby you think of all the things you do now and then repeat the exercise with all the things you would do if you knew you only had had 2 years to live, or what if you had only two weeks, or even less...2 days. When she explained the exercise to me it sounded so much like the type of existential exercise I find so very powerful.

I sat down and tried to put down on paper how my life would change if I had a pre-determined, set time to live. The first thing I thought of was I would call my sisters and express to them how much I love them, the same with my nieces. However, I already do this on a very regular basis, so nothing would really change there. My next thought was that I would feel intense relief knowing life would soon be over. I decided, for me, nothing would change.

I would continue to wait to die, just like always. Dr. X. said maybe we are all waiting to die, but in between we can do things in our life to make the waiting less difficult. I know what he was trying to say...we all die, but we all can impact the time in between, even if in small ways.

I'm sitting there looking at him thinking..."what the hell do you know about waiting to die? You have an interesting job, that you are good at and well compensated for. You have kids to live for and love, you have the energy to ride your bike and to play the piano and to enjoy living."

I know that sounds harsh...but I just felt an intense disconnect between my situation and his. His is the situation I worked so hard to get into and failed so miserably at. I may not have been able to be a Dr., but I had the skills and knowledge to do something with my life that mattered, something I loved: then my lifelong anxiety kept increasing and it and my depression just took me down. Maybe I should be grateful that now I am only "waiting" and not "willing, or making plans on how to make myself die.

That is what I feel Prozac has done for me; changed my wanting to kill myself into, wanting to die. Something I have felt for so many years I have a hard time remembering any extended period where I did not feel like I wanted to die. Is it worth it?

I've been listening to some of my old cds and my whole headspace sort of reminds me of that Smiths song "That Joke Isn't Funny Anymore" :

"Park the car at the side of the road
You should know
Time's tide will smother you
And I will too
When you laugh about people who feel so
Very lonely
Their only desire is to die
Well, I'm afraid
It doesn't make me smile
I wish I could laugh
But that joke isn't funny anymore
It's too close to home
And it's too near the bone
It's too close to home
And it's too near the bone
More than you'll ever know ...

...[refrain]

...It was dark as I drove the point home
And on cold leather seats
Well, it suddenly struck me
I just might die with a smile on my
Face after all

Knowing when I was going to die would be like putting a smile on my face. Life seems to be the big joke I'm not finding funny anymore. Well, really, I haven't found the joke funny for a long time.

Then, after my pdoc appointment, in case I needed another reason for staying in bed, I was the lucky recipient of a $196.00 speeding ticket; all because my road rage kicked in when the twit in the LEFT (passing lane in Canada) lane was dawdling at 40kms/hr in both the passing lane and a lane that was 50 km zone, and crossing the centre line between the two lanes.

I decided the person was going to cause an accident and I needed to get out of the way, so I saw a break in traffic on the right side, slid in and hit the accelerator to pass the idiot's car quickly. I guess a little too quickly, because as soon as my overdrive kicked in an "insane" policeman stepped out right in front of me on the highway...he's lucky I was paying attention as he was pretty close to me when he stepped out...anyways, to make a long story short it ended in him giving me a ticket with a huge fine and me saying "thank you"...can you believe that???

Who's the insane one? Oh yeah that would be ME!!!!...I bet he walked away laughing at me saying thank you for his ticket writing skills...Oy! I should have stayed in bed.

I Am Cycling Downwards...Again

These past three or four weeks I really thought my pdoc and I had hit upon the magic mixture for my depression. For the first time in years I felt like myself for a few weeks. This week that all began to disintegrate once again.

I went to my pdoc appointment today and I felt like I just didn't want to talk. I feel frustrated, annoyed, irritable about the loss of my creativity, high mood, and recently rediscovered feelings of self esteem. All week I have become increasingly fatigued; feeling so tired by 4-4:30 pm that I feel sick I am so tired.

I have been crawling into bed at 4:30 or 5 and sleeping until 7 or 7:30. When I get up I feel less tired, but my mood is still low. The I have not been able to sleep during the night. I am awake, asleep, awake, asleep, awake, asleep...all night long.

Dr. X. suggested I try napping for only 1/2 hour around 4:30 and then get up and go for a brisk walk around the farm. I told him I would try, but today I set my alarm and when it was time to get up I shut it off...I just couldn't get out of bed. I felt like I was made of unmoveable stone.

He also explained to me that I need to try eating small amounts of food throughout the day. In the last 3 weeks I have eaten very little, because when I am well it is like I forget to eat...the rest of life becomes to exciting. I've lost 6-7 pounds, which to me is a good thing, but Dr. X. says nutrients provided in regular eating of food can affect mood.

I did not have the energy, or the motivation before to cook dinner...I have really not been eating well for quite a while, and then when I felt good I didn't want to waste my time cooking. I would say I was eating okay though. I had a bowl of granola/muslix at about 10:30a.m. and then around 7:30pm I made myself a yogurt, blueberry, mango, banana and blueberry juice smoothie. At least I'm getting some fruit. Sometimes I'd eat a couple eggs too.

Anyways, the other thing I am going to try to do (on Dr. X's advice) is actually two things: keep painting even if my creativity seems to have disappeared. He says to keep doing the things that make me feel good when well, even if they are hard to do, or I don't feel like it. Secondly, give myself permission to slow down and ride out the low cycle. He expressed that he knows these are both easier said than done. I appreciate that he gets that.

All in all my mood is lower, but no where near as low as it sometimes goes, so I'm hoping I can get a hold on it before it really crashes. I hate the unpredictability of my mood.

I Wish I Would Die

Caution: Open discussion about suicide, may trigger some people. (Please read this first)

This is just a rant. It's how I'm feeling, thinking, how I am this week. I will survive. I just need to get all this out of me and my pdoc's away.

The short version: My mood is cycling into an extreme low again. I have hoarded medications, including a HUGE amount of heart medication and phenobarbitol I managed to get at the pharmacy for my dog, Bert's, cardiomyopathy, low blood pressure and the seizures he's been having.

I talked the pharmacist into giving me months worth of his meds. I want to die. That was my intention when I picked Bert's medication up. I can't take this anymore...I'm hanging on though. Not sure how, or why.

The Long Version:

I am severely depressed again, after feeling a bit of a lift in mood for a few weeks. My pdoc is away for three weeks. It is the first decent amount of time off I have seen him take since I met him 6.5 years ago, so he deserves it, but I still miss the sessions and feel like I really need someone to talk to right now. I may not seem to get better with therapy, but I find his support each session invaluable.

I have no energy, or motivation. All I want to do is hide under the covers all day.
I have obssessive suicidal thoughts much of the time. Even when my mood lifted for a while I was having thoughts of suicide. In the past I have never acted on these thoughts, although I have come very close. I plan, get all the things in place to do it, write the note, and then, while writing the note I think of how devastated I would be if one of my sisters committed suicide. At that moment I curl up into a ball and try really hard to get rid of the thoughts and to hang on.

Dr. X once gave me a metaphor for hanging on, and I use it when I feel like this. He told me to think of myself being on, and a part of, a strong and sturdy ship in a stormy ocean. The wind and waves (like my depression and all the bad thoughts) may batter this ship, and push it off course, but it will not sink. My job is to hang onto the rudder and steer in spite of the storm. I find this a powerful image. So far I have hung on.

My suicidal thoughts differ depending on the symptoms of the depression. If I am extremely anxious or irritable they are often more violent in nature. When I am in an anxious, raging mood I have images of stabbing myself over and over again, or jumping from a tree out back and hanging myself, or shooting myself in the head.

All these thoughts act like some kind of release for me. The stabbing presents me with images of all the blood flowing out of me, and with it all my sadness. The hanging seems to be an image to snap me back into reality, and the gunshot provides me with a violent image to stop the rage. Dr. X. likens my thoughts to cutting. The thoughts provide me with some form of release.

For years I have horded all my extra medications so I will have a way out if I cannot manage anymore. My dillemma has always been that I do not want to use medications prescribed by Dr. X., because I respect him too much to kill myself using what he has provided me, to help me. Due to this I have always given in and brought in my piles of pills to Dr. X.

This changed recently, because my dog is so sick that he requires heart meds, pain meds and seizure meds...so I now have collected a massive amount of phenobarbital, sotolol, and lotensin for my dog...but I purchased the medications in quantity, so I could use them to go if I can't take it anymore. So I now have the means I need to disappear. I wish I had the will to make that happen.

Why?

Because I can't take not getting better for any reasonable amount of time. A couple weeks a year is simply not worth living for.

Because I am trying so hard to get better and nothing works, not medicine, not therapy, not my volunteering and trying to do some resembling work.

Because, all that ever seems to happen when I begin teaching again is I get so anxious, stop sleeping, stress out about how the classes are going, obssess about all my mistakes, obssess about why I cannot engage everyone, obssess about how I cannot do this, obssess about whether or not I am helping people...I feel like I'm trying so hard and it just isn't working.

Because, all my husband and I ever seem do do is avoid each other, or fight. I try so hard to get along, but he keeps telling me I just have to stop being so angry, just stop being depressed, or sad, or anxious...and I don't know how to do that. I'm trying, but nothing works. I hate who I have become.

I hate myself so much. I do not understand why this is happening to me. I do not understand how I worked so hard my whole life to get through university, to develop myself in my career...and now I'm nothing. Nothing, but depressed. Where did I go?

Early Medical Intervention For Major Depression

I am sure I am not unlike many people with treatment resistant depression insofar as I am constantly wondering WHY? Why is no medication working? Why does therapy, while being supportive and managing to keep me here on this earth, not help me become well? Why do others get well with medications, or ECT, or therapy, while I continue to struggle?

I recognize there are no hard and fast truths to any of these questions. I do however, have theories about my own circumstances.

One theory is that I was not diagnosed, and "fully" treated for depression until I was 36 years old. Now that I understand what a depressive episode is I definitely had severe depressive episodes as early as 17 or 18 years old. From what I know of my history I had at least 7 or 8 major depressive episodes, lasting anywhere from 3-4 months, up to two years before my primary care physician recognized I was depressed and needed a referral to a psychiatrist. I moved around a lot, so I saw many family physicians between 18 and 36, it was my current Dr. who saw I needed help.

I say "fully" treated because between 19 and 36 I saw at least 6 different psychologists. Not one mentioned depression was an illness, or that there were medications to treat depression, or that I was having a major depressive episode.

The first I saw was during my first year of university when I was 19 years old. This psychologist told me my thoughts of suicide were because I did not have a clear "career path". They had me do career testing, which showed the profession I should choose was in the corrections system, that of a "Prison Warden" was at the top of the list. If you knew me you would see that this should have raised red flags. I am so left leaning that I do not even believe in the corrections system. I think this career came up because I was having severe "black and white , all or nothing thinking". Also, my father was a policeman, so I grew up in a world that embraced the corrections philosophy. The psychologist never once explained I was depressed, or that depression was a treatable illness.

Three years later I returned to university. That year I had another severe major depressive episode (MDE). I was suicidal most of the time, had a plan in place, visited the place I was going to commit suicide numerous times, found myself crying uncontrollably in class, was unable to do any assigned work, and was completely distraught for no apparent reason. I am not sure how I managed to take myself to the university's counselling department, but I did. I saw a psychologist who really helped me using cognitive behavioural therapy. She was very helpful, but again never spoke to me of depression as an illness, or told me about medications that might help.

For the rest of University I was well and upon leaving school, desperate for a job to pay off my student loans I took a position in a large corporation. Almost immediately I knew I had done the wrong thing. I did not belong in this kind of job. I owed $30,000 though and I felt trapped by my debt. Problem was, I was good at my job and I kept getting promotions. With each promotion and wage increase it became more and more difficult for me to extricate myself from the organization. Not because I was greedy, but because of my intense need to feel feel financially safe. Also, contrary to reason, with each promotion my self esteem seemed to sink and I feared I would be fired, or would never find another job.

In my 11 years working for that corporation I saw three different psychologists for three separate MDE's through the companies employee wellness program. I did not do well with any of these psychologists. During treatment not one of them mentioned depression as an illness, or that I may have an illness, or that medication might help, despite my repeating my depression history to each of them.

During my fourth MDE at the organization I began having anxiety attacks. I had been an intense "worrier" since as young as I can remember. As early as 9 I used to sneak into our medicine cabinet and take whatever I could find to help me sleep (neo citron, cold medicine, 292's, etc.) I could never seem to sleep because I worried about everything. This was different though, I began to feel like I could not breathe when I was having what I now recognize as anxiety attacks.

I sought out an external psychologist. I saw her for therapy once a week, for two years. It did not help at all. I gave up. Again, no mention of depression as an illness.

I was feeling so desperate. One day I was in a bookstore and was looking at books on the psychology shelves and I stumbled on the book "Listening to Prozac". I bought it and read it. It was the first I had ever heard of depression being a bona fide medical illness. I had never heard of anti depressants. I was dumb founded.

In the book the author discussed a theory about the "kindling effect" of untreated depression. His theory was that with each untreated MDE the episodes become longer, harder to treat and closer together. This has been my exact experience.

Why had no one told me about this? Maybe I was stupid for not figuring this out earlier on my own, but this was before there was really anything being written about antidepressants in the newspapers and the professionals I saw for help never mentioned a thing about medication.

After reading a few more books about antidepressants, some pro, some con, and some by people struggling with mental illnesses, at 33 I went to my family doctor and asked her to give me an anti-depressant. We tried two different ones. They did not help.

I went back to my Family Doctor and she referred me to an outpatient program at the hospital. They had me join a class that taught cognitive behavioural therapy and then had me join a 3 month psychodynamic group therapy group. Despite both groups being taught as part of a psychiatric program there was, from what I recall, NO talk of medications being an important part of fighting depression. At the end of the psychodynamic group I was still severely depressed and quite vocal in the group about having no support once the group ended. The psychiatrist leading the group knew I was still depressed. Yet, no one followed up with me after the group ended.

A few months later I became spontaneously well for approximately a year and a half. I began slipping into a severe depression the summer of my 36th year. I went to my Family Doctor again and she finally referred me to a mood disorders clinic, where I met, and began seeing, a private psychiatrist for the first time. He is and was a godsend.

Now, six plus years into this MDE I'm still searching for something that will help me, but I believe I at least FINALLY, in the psychiatrist I see, have the knowledgeable and completely supportive help I needed all along.

I often wonder, had I received help at 18 or 19, instead of 36, would I be better today? Would I be struggling so hard to find something to help me?

The Mood Whose Name Cannot Be Spoken

I'm afraid to say it, because everytime I do it seems to jinx how I am feeling. I am feeling that mood that begins with "W" and ends in "L"; otherwise known as feeling that word that begins in "G" and ends in "D".

This feeling has come about very quickly, like my switches in mood from severely depressed to almost hypomanic. According to my pdoc though it is a medication induced switch. I have been taking Tegretol (carbamazepine) for the last few months to stabilize my mood. It is so great in that it has had no negative side effects and had been helping me sleep without feeling at all drowsy. That great sleep you get where you , fall asleep easily, dream and upon wakening often remember your dreams. I love that.

The Tegretol helped stabilize my mood a bit, but I still felt severely depressed, so two weeks ago my pdoc added prozac at my request. I had tried all the other SSRI's, but not this one. I never had any luck with the other SSRI's and they made me so fatigued/tired I could hardly move (although that could have been the continued depression).

Within a week and a half on the Tegretol/Prozac combination I began feeling a change; more energy, elevation in mood, more interest in life and those around me and I felt more attractive, even magnetic. God I even asked my husband for sex. Sex has been really lacking in the past year. I am usually highly sexual, so this was a good sign.

There were a few problems with this mood switch. I have developed what my pdoc called a "tic". I have relentless snippets of music; four or five bars, playing over and over and over inside my head. Attached to this is a constant "clicking" or "tapping" of my teeth to the music. It is not a conscious thing. If I try to stop my teeth from clicking to the beat they stop while I am thinking "stop", but I quickly lose focus and they begin again. The music is even less responsive to my will. It simply will not stop.

I also find myself having way too much internal energy. A sensation that I am racing. Not neccesarily my thoughts, but a physical sensation that I am really speedy inside. I find myself unconsciously tapping my fingers, my leg is shaking like I have to much energy or am restless or agitated and I have an increase in my heart rate. My regular resting rate is 60 bpm. When I have measured my resting pulse rate while I feel like I am racing inside my pulse rate has been 70, 75 and 78 bpm.

This change quickly led to my craving alcohol again to slow myself down...(why do I do this)...so I began to drink again; drink, take valium and take muscle relaxants to try to calm down and slow down. This should be so predictible to me. Every single time my mood starts to go high I begin to drink/take benzodiazepines. I cannot manage the intensity, anxiety, agitation and speed of this mood state.

I told Dr X. about my self medication (thank god I trust him enough to be honest). I also expressed to him that I am afraid this mood that cannot be spoken is temporary, and, like my mood switches before, will disappear, or is precarious. He said it may be precarious, and may become lower, but he does not think it is like my switches before. He said the quickness of the switch; the fact that my mood responded to the antidepressant so rapidly, points to a bipolarity in my mood disorder.

I asked him if maybe the rapid switch was a placebo effect, in the same way that I think maybe my rapid response to ECT was, or how I seemed to respond to Lithium for 5 weeks after I was on 1200 mg and then lost my good mood entirely. He said no it wasn't a palcebo effect. That a placebo effect would not come with the particular neurological effects I am having (the tic). Maybe he is just saying this to keep me hopeful...but if so it worked. I do feel hopeful. Another sign that I am feeling w**l.

Yesterday Dr. X. prescribed Clonidine to me to try to help me sleep and to stop the music and teeth clicking tic. He mentioned it was used for helping people with heroin and smoking addictions come off those drugs...so I think maybe he is hoping it will help me stop drinking and taking valium. As I left yesterday he did quietly suggested that maybe this medication will help me slow down and sleep so I do not feel the urge to slow down in other ways.

I took the Clonidine last night, and I became sleepy about an hour later and soon fell into a deep and restful sleep. I woke up this morning and the music and teeth clicking was gone. Unfortunately it returned shortly thereafter, but I am hopeful it will go away over the week.

I had a great pdoc appointment yesterday. I felt cared for and listened to and it is always an incredible experience to spend an hour with Dr. X when I feel that word that cannot be spoken. Here's hoping we have hit upon the right combination of medications.
